By registering on VIVID, parents and guardians agree to the following Terms and Conditions:
1. Accuracy of Information
Parents certify that all information provided during registration is true, complete, and accurate. Any falsification may result in suspension of access or cancellation of enrollment.
2. Consent to Data Processing
By registering, parents consent to the collection, storage, and processing of their personal and child’s information for slot reservation, enrollment management, billing, assessment scheduling, and communication purposes, in compliance with HIPAA and RA 10173.
3. Slot Reservation and Waitlist Notice
Clients who submit a slot reservation or waitlist request will be contacted once a slot becomes available. Please note that filling out any reservation or registration form does not guarantee immediate placement. Slot availability may vary depending on current capacity, and the center will do its best to accommodate families as soon as possible.
4. Account Responsibility
Parents are responsible for maintaining the confidentiality of their login credentials. Unauthorized sharing of accounts is prohibited.
5. Payment and Billing
Parents agree to comply with the center’s payment policies, including timely settlement of fees. Proof of non-cash payments must be submitted through the system for validation by the Finance Officer. Parents acknowledge that the validation of submitted payments may take one (1) to three (3) business days, after which their payment status will be updated in the system.
6. Client Requests and Reports
Parents may request documents such as evaluation reports, progress reports, certifications, referral letters, quotations, SOA, and invoices. VIVID reserves the right to set processing timelines and delivery methods (email or pickup).
7. Assessment Scheduling
When a parent reaches the first position in the queue, VIVID Center will notify them via Messenger or Viber to confirm availability for an assessment slot. Upon confirmation, the parent and VIVID will agree on an assessment date. Parents who remain unresponsive for 2–3 days will automatically have their slot voided. If a parent needs to reschedule, they must notify VIVID Center directly. Rescheduling will only be allowed for valid reasons; otherwise, the slot will be voided. A no-show on the scheduled assessment date will also automatically void the reserved slot.
8. Confidentiality
All submitted information, including health-related data, is confidential and protected. VIVID will not disclose personal data to third parties without consent, except as required by law.
9. System Use
Parents agree to use the platform responsibly and solely for legitimate purposes. Misuse, including fraudulent requests or unauthorized access, may result in termination of access.
